  Unfortunately, it doesn't seem to be working, as no record is created in my system. Also...it should take you to a thank you page after you hit submit.
  I have tried this in multiple places on the site and the only way I can get a form to work is if I first have a new window open that is a black HTML page with a form on it. I really want to figure out how to do it directly on the site though so I can use a form as "Pre-Content HTML" block for specific products.

                It looks like the test is gone, but I will guess that you wrapped your form in <form> tags. That will not work. ASP.NET only allows one form on a page. If you try to insert or append a second form, it will at least confuse ASP.NET and at worst cause an error.
  You can put your form inside of an iframe.    |
